information. The data is submitted by the practitioner and has not been verified by the Department unless
otherwise indicated. The physicians file the data with the Division of Medical Quality Assurance as
required by law. The 5 professions required to provide this data are Medical Doctors, Chiropractors,
Osteopathic Physicians, Podiatric Physicians, and Advanced Registered Nurse Practitioners. Responsibility
for providing the data lies with the practitioner.

text. What do I do?

You must first save the data as a text file (.txt), and then import it into a spreadsheet program like Excel

I get an error when trying to download files from the Licensure Data File Download Page. What can I do?
The Department of Health's download site is secure and requires what is called a "reverse DNS lookup"
before allowing a connection. Most errors with downloading from the Licensure Data File Download Page are because the
download site cannot do this. This is an issue that needs to be addressed by your office or internet service provider's
(e.g. Comcast, Sprint, AOL) technical support staff. Please check with them to verify your DNS settings. Inform them
that you are trying to download files from an ftp site that requires reverse DNS lookup of your host.
